Dear Patients;
The Moose and Squirrel Medical Clinic has always based our practice around the “Patients Medical Home” model. As your Medical Home we are focused on offering the care you need, in your community, when you need it. Ensuring accessibility to care and continuity of care remain our top priority.

Moving forward the Moose and Squirrel Medical Clinic will be transitioning to a new funding model called the Blended Capitation Model. We are excited by the options this new model provides us for continuing to provide excellent, comprehensive care to all our patients. As well, many physicians are interested in working in clinics using this model and we believe it will allow us to encourage new doctors to join our practice so we can provide care to all our current patients and eventually offer care to others who still need a physician.

One of the requirements of us moving to this new model is to clearly define who are patients of our clinic, and this is done by having each patient sign an affiliation form. This allows us to continue to provide you care at the Moose and Squirrel Medical Clinic.

By signing this affiliation form you agree to contact us first for your medical needs and update our clinic, your Medical Home, of any changes to your health. The signed affiliation form will attach you to a Family Physician. We recognize urgent needs will arise, our 24/7 emergency telephone line is available (587-444-6762) and as always, Emergency Room visits will remain available to Moose and Squirrel Medical Clinic patients.

This also means that if you access care elsewhere (i.e. another clinic) we are notified, and this pulls funds and resources away from your Medical Home. Every possible opportunity to provide care for you at the Moose & Squirrel Medical Clinic will be available to ensure you can be seen as needed.
Referrals for specialist appointments will continue to proceed as usual. Signing the affiliation form will not restrict your access to diagnostic imaging or laboratory services provided by Alberta Health Services.
