When you receive health services from the Moose & Squirrel Medical Clinic, we will be collecting individual identifying personal and health information and will share this internally as well as with other health service providers that are involved in your care. This is done to ensure you have the best possible medical care.

Your personal health information is collected, used and disclosed in accordance with the provisions of the Health Information Act (HIA) and is primarily used to provide diagnostic, treatment and care services to you, and to bill Alberta Health and Wellness for services provided.

The privacy provisions of the HIA require that the Moose & Squirrel Medical Clinic protect your confidential health information from unauthorized access, use, disclosure or destruction.

However, due to the nature of electronic communications (email), it is important for users of this website to understand that we do not guarantee that information submitted by email will not become known or accessible to third parties.
